24-25 Sixth Grade Parents
As you know, all students entering 6th grade are required by law to provide the school with a new physical examination, dental examination, and immunization record. Please return your child’s completed physical examination, immunization record and dental examination to the school health aide by June 1st. Students who do not provide a complete and current physical and immunization record prior to the first day of school will NOT be allowed to attend school. If you have any questions, please contact your school health aide.
